Late Cancel / Missed Appointment Policy

$100 fee will be charged for each Well Child Exam, Behavioral visit (i.e. ADHD/Anxiety/Medication), all Chronic condition appointments- all visits 30 minutes or longer. 

$75 will be charged for each other Appointment type

Fees are charged per patient appointment with Provider.
All appointments must be cancelled during business hours by phone only.

The following appointment will be charged a fee: 

1. Appointment scheduled before the day of the appointment cancelled/missed within 24 hours.
2. Missed appointment scheduled same-day 
3. Same Day Appointments “late-cancelled” within 1 hour of appointment 
4. Same Day Appointments scheduled in the AM for after 2pm, not cancelled before 1:00pm will also be charged a fee.
5. Fee will be charged if you arrive tardy (past 15 minute grace period) and Provider is unable to accommodate if it delays other patients’ appointments.
6. Due to large volume of patients missing appointments at our office, unless there is a true family emergency, fees will not be waived for any other reason (i.e. fees not waived for forgetting, not receiving reminder text, has an exam in school, has band/sports practice, work conflict, miscommunication between parents, etc…)
7. Habitually missed and/or late-cancel appointments, family will be subject to practice dismissal
